Farewell to Family Medicine
Dear Patients and Friends, With a full heart, I want to let you know that I will be retiring from the practice of family medicine after 36 years of service. Sterling Family Practice will be closing its doors. This is not an easy goodbye. Being your doctor has been one of the greatest honors of my life. I’ve had the privilege of walking with many of you through the most personal and meaningful moments—welcoming new life, managing illness, grieving loss, and everything in between. These connections have shaped me in ways I will never forget. It’s difficult to put into words how much I will miss seeing you—hearing about your families, your milestones, your daily lives. I’ve come to know so many of you not just as patients, but as people I care about deeply. Letting go of that day-to-day connection is not easy. That said, I’m incredibly grateful to be leaving you in the capable hands of Nurse Practitioner Cynthia Yew at Sterling Wellness. Most of you know her already, and I know she will continue to provide the thoughtful, compassionate care you deserve. Her office will remain at our current location at this time. Upon my leaving, Cynthia Yew will become the official custodian of your medical records. If you wish to transfer your record to another medical provider, you will need to complete a HIPAA compliant release form and we will transfer the records to you, or to the provider of your choice. You can request a release form by sending an email to info@sterlingwellness.com or by downloading a form from our website at sterlingfamilypractice.com. Click on the FOR PATIENTS tab and then FORMS, then AUTHORIZATION FOR RELEASE OF MEDICAL INFORMATION. Once we receive the signed form, we will transfer your records as requested. Please complete the form and return it as soon as possible. Rest assured that our office will be available to answer any questions you may have and promote a smooth transition and continuity of care through this change. My last day in the office will be Thursday, September 18, 2025. The last official day for Sterling Family Practice will be Friday, September 19, 2025. Between now and then, I hope to see many of you, and I welcome the chance to say goodbye in person. I will be taking a much-needed rest and will be following Christ’s plan for the next chapter of my life. Thank you, from the bottom of my heart, for the trust, the kindness, and the many years we’ve spent together. I will carry these memories with me always. With deep gratitude and affection,
Sarah S. Fletcher, M.D.
